Information you provide
When you use the appointment request form, you may provide your name, email address, phone number, and a short message. This information is sent to Frank by email solely so he can respond to your inquiry. It is not sold, rented, or shared with third parties for marketing.
A note on email security
Messages submitted through this form are delivered by email, which is not a fully secure or encrypted channel. Please do not include sensitive personal health information, insurance details, or anything you consider confidential in your message. An initial phone or secure conversation can be arranged for anything sensitive.
Form processing
The contact form is delivered using a third-party form service (Formsubmit) that transmits your submission to Frank's email. That service processes your message only to deliver it and is subject to its own privacy practices.
Analytics & cookies
This site does not use advertising cookies or sell visitor data. If basic, privacy-respecting analytics are added in the future, this policy will be updated.
Your choices
You may request that Frank delete any message or contact information you've sent by emailing Divenuto.psychotherapy@gmail.com.
